SUMMARY DESCRIPTION OF WORK
Intense therapeutic responsibilities begin at this level. Educational requirements are very specific. Decision-making skills and problem-solving capabilities are critical. Professionals at this level have excellent communication skills with both client and community institutions. Specific project, program or office responsibilities may occur at this level. This position functions as a fully integrated Team Member of the Assertive Community Treatment Team, which is a multi-disciplinary team, that assists individuals with a serious mental illness to minimize symptoms and the resulting impairment in daily functioning. Highly developed interpersonal skills and case management capabilities must be present.

MINIMUM QUALIFICATIONS
- Bachelor’s degree in mental health, social work, or health related required. Masters preferred.
- CADC and/or LADC required, LISW or LMHP with IADC preferred.
- Valid driver’s license, acceptable driving record
Essential Duties and Responsibilities
- *Conducts a comprehensive intake interview, focusing on substance use history and treatment.
- *Establishes a treatment plan, with client participation, which specifies treatment structure, outcomes, goals, and objectives.
- *Provides individual and group substance use treatment in accordance with best practices.
- *Provides counseling to individuals, in group or individual settings, within the framework of the treatment plan, monitors progress.
- *Works as a team member with other program staff to provide trauma-informed care to clients.
- Assist other programs in identifying clients who have substance use issues, complete Substance use evals.
- *As a member of the Assertive Community Treatment (ACT) Program is accountable for responsibilities, etc. described in attached job description for ACT Team Member.
- Network with other Substance use providers to develop collaborations that will help clients with co-occurring mental illness and substance use connect with levels of care that are appropriate, trauma informed and engage people with co-occurring issues.
- *Administers clinical measures to monitor client progress and program effectiveness.
- *Is dependable and punctual regarding scheduling and attendance.
- Maintains relationships with referral sources.
- Coordinates care when provided by multiple practitioners.
- Refers to other Heartland Family Service programs and other community services, as necessary either concurrent with services or for aftercare; follows up to complete referral.
- Completes all case documentation for clinical and billing purposes.
- Maintains clinical record, meeting established standards of timeliness and completeness.
- Is prompt and accurate in completing all required reports, statistics and other requirements.
- Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the Agency.
- Cooperates and collaborates with program area staff, volunteers, and other Agency staff.
- *Is dependable and punctual regarding scheduling and attendance.
- Abides by all specific program and Agency procedures, policies, and requirements.
- Develops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media.
- Performs other program related duties as assigned.
