Job Description

PURPOSE OF POSITION:

The Crisis Stabilization Case Manager provides case management services to youth admitted into the Crisis Stabilization Programs. This role works closely with families, staff, and community providers to ensure safe and effective care transitions. It is essential to create a supportive environment that helps stabilize youth in crisis and aligns with individual care plans and treatment goals.

 

ESSENTIAL RESPONSIBILITIES:

  1. Case Management & Coordination:
    • Assess youth needs and develop individualized service plans in collaboration with the family and clinical team.
    • Monitor and ensure the execution of treatment plans, making necessary adjustments in response to the youth’s progress and changing needs.
  2. Collaboration & Advocacy:
    • Act as the primary liaison between families, schools, and other service providers, ensuring all stakeholders are informed and aligned.
    • Advocate for youth and families to access appropriate community-based services and resources upon discharge.
  3. Documentation & Compliance:
    • Maintain accurate records, including progress notes and treatment plans, in compliance with agency, state, and licensing standards.
    • Monitor compliance with all relevant legal, ethical, and program guidelines, including HIPAA regulations.
  4. Crisis Intervention:
    • Provide support during crisis situations to stabilize youth and ensure safety within the program
    • Work with the clinical team to create and implement crisis management plans.
  5. Program Support & Community Engagement:
    • Assist with program development, scheduling, outreach, and continuous quality improvement initiatives.
    • While on shift, serve in ratio at times as needed per program demands.
    • Facilitate group activities or educational sessions as needed.
  6. On-Call Support:
    • Serve in a rotation for on-call emergent situations and questions from staff.
  7. Establish and maintain relationships with community stakeholders, agencies, and officials to advocate for youth and families.
  8. Provide effective leadership by communicating the organization's vision and mission.
    • Collaborate internally to ensure services meet organizational initiatives.
  9. Liaise with internal and external stakeholders, the public, professional community, referral agencies, and other relevant organizations to build and maintain support for programs.

 

OTHER RESPONSIBILITIES:

  • Serve as an excellent role model for staff and residents.
  • Complete training required for the position and as specified in Agency and Program Staff Development Programs.
  • Promote a positive team and work environment.

QUALIFICATIONS:

  • Education: Bachelor’s degree in social work, psychology, or a related human services field.
  • Experience: Previous experience working with youth in crisis, mental health, or residential settings preferred.
  • Skills:
    • Strong communication, organizational, and interpersonal skills.
    • Ability to work collaboratively with families and external service providers.
    • Knowledge of community resources and services for youth.
    • Ability to manage time effectively and handle stressful situations calmly and professionally.

 

Other Requirements:

  • Valid driver’s license and access to reliable transportation.
  • Knowledge of state licensing requirements and crisis intervention protocols.

 

ESSENTIAL PHYSICAL DEMANDS AND TYPICAL WORKING CONDITIONS:

  • Flexibility of hours worked, including evening and weekend work and working additional hours at times.
  • Significant time is spent in the environment in which clients are served.
  • Must be able to move around campus and engage in activities with youth.
  • This role may involve some travel to engage with external service providers and families.
  • The work environment can be emotionally demanding, given the nature of working with youth experiencing mental health crises.
